Entry Form

There are three ways to enter this meet. Please select the method that is most appropriate. We strongly urge you to use the online entries method. It saves you time, and saves us time and typing.

Enter Online
with a
Credit Card

   ONLINE ENTRIES ARE CLOSED. You can enter this meet online through your web browser using a credit card. This option is only available to swimmers registered in the New England LMSC. If you are not from New England, please use one of the entry forms below. You will be required to use Google Checkout to pay for your entry with a credit card. If you do not have a Google Checkout account, you will be able to create one during the entry process. If you have already sent in a paper entry form, please do not submit an additional entry using this online tool.
 
Fill-In Adobe PDF Form
   This form allows you to type your entry information directly into your computer, and then print out the form to mail it in. To use this form, click on the link to the left to open the form on your computer. Then type directly onto the form using Adobe Acrobat Reader, print out the form on your printer, sign at the bottom, and mail it in with your payment. Using this form makes it easier for the meet staff to process your entry. This form will automatically calculate your entry fee for you. (If you will fill-out your form by hand with a pen, please use the other form.)
 
Non-Fill-In Adobe PDF Form
   Use this form if the above form does not work, or if you need to fill out the form by hand. To use this form, click the link to the left to open the form using Adobe Acrobat Reader, and print it out on your printer. Then, fill out the form by hand, sign at the bottom, and mail it in with your payment.

Entry Form Reminders
  • Sign and date your entry form
  • Make sure your date-of-birth is correct
  • Make sure your USMS ID number is correct
  • Include your payment with your entry
  • Make sure your write your name exactly as it
    appears on your 2009 USMS registration
    card (including any middle name or initial)
